Below are some forms for download that might be of use:

  • Youth (cub scout) membership application—We have the hard copy forms also—after completion give to one of us. Note that this is for new cub scouts—if your boy has been with us already, all we need are the annual dues ($50) to renew his membership.
  • Adult membership application—same as above, but no dues.
  • Local tour permit application—this is mostly for leaders, for trips such as camping at council (NEIC) camps.
  • Cub Scout and Webelos uniform inspection sheet—also lets you know where those patches go.
  • Adult (leader) uniform inspection sheet—both men and women.
  • Annual Health and Medical Record—form to fill out if your boy goes to summer camp, etc.
  • Family Talent Survey Sheet—let us know more about yourself.
  • District 65 facilities application—To apply, follow the link and click on the Building Permits tab at the bottom left, where you will find instructions and the current forms to fill out. This is also the place to go for the form to apply for distributing materials (e.g., getting fliers into the "Friday folders"). For building use, the first two forms below are required (note that the NEIC already sends each year the "Certificate of Insurance for One Million Dollars naming D65 as Additional Insured", but questions about that can be directed to the NEIC "Insurance Certificates" person that you can find on the NEIC Staff Directory page—Barbara Ziegenweide at 847-748-9164), but for distributing materials, only the last form is required. These form(s) should be turned in at the District 65 building (Joseph E. Hill Education Center at 1500 McDaniel Avenue in Evanston) or faxed there (fax 847-328-7459), and questions can be addressed to the Information Services Department, Sherry at 847-492-5887. If approved, the details will be postal mailed back to you—allow for 2 weeks for processing.
